Is there a model of this backpack with a waist strap?

Yes! Our K9 Sport Sack Knavigate has a waist belt!

Wondering if this is suitable for senior dogs? Would this cause them discomfort by putting too much pressure on their spine or hips?

You can use it with senior dogs, but may want to speak to your vet to see if it is okay for them to be in.

Will an English cocker spaniel fit in well?

Depends - my dog is about 20 pounds, so if yours is that or less, then probably.

My dog measured 16.5-17" from base of his tail to collar, and 18" around his chest so I went with the medium. It seems maybe too long, how can I tell?

The medium is usually for dogs 17-20 inches in length. You could add a booster block so it takes him up in height.
